If you have or know of a youth where there are concerns related to emotional/behavioral health, a developmental/intellectual disability, or substance use treatment is needed, the first thing to do is contact PerformCare, New Jersey at 1-877-652-7624. PerformCare partners with the state of New Jersey's Children's System of Care (CSOC) and is the single point of entry for youth and families in need of resources and services.
Youth who are eligible for services through PerformCare are primarily between the ages of 5 and 21 and reside in New Jersey. Special consideration may be given for youth under the age of 5. When contacting PerformCare, the parent or legal guardian of the child must give consent for any services unless the youth is over the age of 18.
There is no charge for calling PerformCare. Any service that may be recommended through PerformCare may be be authorized without regard to income, private health insurance, or eligibility for Medicaid/NJ FamilyCare or other health benefit programs.
Next Steps....
Once a youth or caregiver is connected with PerformCare a series of questions will be asked and based on the information provided, the youth and caregiver will be connected to the appropriate resources/services. A clinical professional may be assigned to meet with the youth and caregiver to complete an assessment to determine a level of care and which services will be linked to the youth. This may include the youth being referred to a Care Management Organization (CMO) in the youth's county of residence.
Partnership for Children of Essex (PCE) is the Care Management Organization for Essex County and will follow up with the youth and family within 24 hours of a referral being made.
If there is an immediate crisis or concern, the youth and caregiver will be connected to Children's Mobile Response and Stabilization Services.
It is important to keep in mind that any services offered through CSOC are confidential, strengths based, and family friendly.
