Program/Service Structure
- Population(s) served– Partnership for Children Essex (PCE) serves youth with behavioral, mental health, substance use, intellectual, and developmental needs primarily between the ages of 3 and 21 and their families.
- Settings– Care management service settings vary depending on the youth and family’s choice. Services are offered in the youth and family home, in the community, and/or at the offices of Partnership for Children of Essex.
- Hours and Days of services– Office hours are 9am-5pm Monday – Friday. Services are provided during times that best meet the needs of the youth and family which may occur outside the office hours. For emergency and crisis situations, youth and families can access PCE on-call services outside of office hours during the week and on the weekends.
- Frequency of services– Care managers meet with the youth and families in person twice a month and are in contact with the families weekly.
- Payers and funding sources– PCE care management services are funded by Medicaid and Department of Children and Families Children’s System of Care (CSOC) contract dollars.
- Fees– Care management services are free to youth and families.
- Referral sources– Referrals to PCE care management services are primarily from parents, schools, DCPP, and Juvenile Justice via contacting Perform Care, Children’s System of Care (CSOC) Contracted System Administrator, and Children’s Mobile Response and Stabilization Services (MRSS).
- The specific services offered– PCE provides care management services directly to eligible youth and families in Essex County, NJ. Care management services provide linkage and care coordination to community-based services that address identified youth and family needs.
Sharing of Information Regarding PCE Services
- Youth and Families– Each enrolled youth and family is provided with an Initial Folder that review scope of services. The care manager also reviews the scope of services with the youth and family as often as needed. The scope of services is discussed at Child Family Team meetings which include persons served.
- Families/support systems– Enrolled families are provided with an Initial Folder that review scope of services. The care manager also reviews the scope of services with the youth and family as often as needed. - Referral sources– Scope of services is available on the PCE website and PCE brochures.
- Payers and funding sources– Scope of services is available via the contract between PCE and DCF/CSOC as well as the CSOC issued care management handbook.
- Other relevant stakeholders– Scope of services is available on the PCE website and PCE brochures. Additionally, PCE offers presentations regarding the scope of services to any interested stakeholders such as youth and families served, community-based providers, and CSOC system partners.
- The general public– Scope of services is available to the general public on the PCE website, Facebook, and Instagram. Additionally, PCE offers presentations regarding the scope of services to any interested parties.
